  1. Inform Sophie as soon as possible to open a file with the insurance company,

by email or by phone at 02/414.28.73.

Procedure

  1. First, fill out the forms together with your doctor.
  2. Then return these documents to Sophie (by email or at the club) — don’t forget to sign and attach your health insurance sticker in the designated area.

The first section of the first page must be completed by the club.

Your Health Insurance Fund (from €15 to €50)

Depending on your health insurance fund, there is a fixed annual reimbursement (from €15 to €50 per year) for the sports activity followed by your child or yourself.

=> Print the invoice from your Member Area and submit it to your health insurance fund to receive your reimbursement, if applicable.

Vouchers – CPAS (up to 80% of the costs)

If you meet the eligibility conditions set by your CPAS (Public Social Welfare Centre), you may receive a reimbursement of up to 80% of the costs.
Please check directly with your local CPAS, as each one has its own reimbursement policy.

This support can also be combined with the reimbursement from your health insurance fund.
